Available forms include:
Indian Boarding Homes
• Category 1 – Compensation for placement in an Indian Boarding Home (Individual Claim)
• Category 2 – Compensation for abuse (Individual Claim)
First Nations Child and Family Services & Jordan's Principle Settlement
• Form A – Removed Child Claim
• Form B – Representative of a Removed Child Who Has Died
Birth Alerts BC Settlement
